TAMPA, FL, UNITED STATES, September 15, 2026 /EINPresswire.com/ -- Today, HHS and CMS leadership join national healthcare leaders for Building a Healthier America: CMS ACCESS and a New Chapter in Health Innovation, coinciding with the scheduled launch of the ACCESS Participant Directory. Against that backdrop, HealthyConnect is announcing its participation in the ACCESS Model through Remote Care Partners, the legal entity participating in the model, and its plans to expand technology-supported chronic care for Medicare patients.
